We are seeking a Full- Time, 40-hour LPN to support our practices onsite Monday through Friday from 8:30am to 5:00pm. We believe in providing exceptional care and that starts with exceptional people. We're seeking an LPN to join our collaborative, patient-centered team! Dedicated to clinical support, our nurses provide professional nursing care while collaborating with providers, staff, patients and families. This role is a mix of both patient facing and non-patient facing responsibilities. This is a unique opportunity to work autonomously as well as collaboratively with our team of 5 medical assistants. Responsibilities: * Responds to and prioritize incoming telephone calls, and triage appropriately. * Communicates with patients and family regarding medications, test results, and treatment instructions. * Vaccine/Medication administration and Management -Vaccine inventory * Population Health Management/Health Maintenance- Chronic Disease mgmt. * Observes, records and reports patient's condition and plan of care to the physicians/provider. * Manages all aspects of process Prior Authorizations for: Rxs, DMEs, procedures, labs. * In-Basket management. * Ownership of Nurse Resource schedule for vaccines, BP checks, etc If you are someone who values meaningful patient interactions, excels in a dynamic clinical environment, and takes pride in ensuring smooth and efficient patient care delivery this is the role for you!
