Description Summary: This leadership role oversees a hospital-based Post Acute Paramedic and Convalescent Transport service lines, both designed to reduce readmissions and improve transitional care. The Manager is a seasoned paramedic with deep clinical experience and proven leadership skills, responsible for strategic planning, regulatory compliance, staff development, and operational excellence across two service lines: Post-acute care for discharged patients with chronic conditions (CHF, Sepsis, Diabetes, Pneumonia) and Non-emergency transport of stable patients to skilled nursing or rehab facilities. Responsibilities: Strategic Leadership * Design and implement integrated care models that align with hospital goals for readmission reduction and value-based care * Develop performance metrics and dashboards for both service lines * Lead cross-departmental initiatives with case management, discharge planning, and outpatient services Regulatory & Clinical Oversight * Ensure compliance with CMS, NCOEMS regulations, protocols, and hospital policies * Maintain licensure standards and documentation for paramedics and EMTs * Oversee infection control, patient safety, and clinical protocols Workforce Management * Supervise and mentor a team of Post Acute Paramedics and EMTs assigned to Convalescent Transport * Manage daily schedules, timekeeping, and verify payroll * Conduct performance reviews, maintain training records, credentialing/renewals, and continuing education Operational Excellence * Coordinate schedule, routing, and logistics for mobile units and convalescent transport * Oversee fleet maintenance, equipment readiness, and supply inventory * Implement EMR documentation workflows and mobile documentation standards * Develop and implement patient feedback forum for quality/satisfaction Stakeholder Engagement * Serve as liaison to hospital leadership, Home Health agencies, skilled nursing facilities, and other community partners * Lead collaboration with physician practices and clinics to enhance CPP visits * Lead collaboration with nurse navigators to improve continuity of disease specific care Represent the program in regulatory audits and quality improvement committees * Coordinate with discharge planners, case managers, and outpatient providers * Represent the program in hospital committees and community forums. Other information: Other Information Education High school diploma or equivalent Licensure/Certification Licensed Paramedic. Basic Life Support (BLS) required upon hire. Advanced Cardiovascular Life Support (ACLS) Pediatric Advanced Life Support (PALS) are required prior to completion of orientation. Experience At least 3 years of experience as a paramedic, preferably in an emergency department setting. Experience in a leadership, supervisory, or critical care role strongly preferred. Knowledge, Skills and Abilities * Excellent communication and interpersonal skills * Strong organizational skills and attention to detail Valid NC Driver's License: Yes If driving a Wayne UNC Vehicle, must be 21 years old and MVR must be approved by Risk Management.
