The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The Medication History Technician (MHT) shall participate in conducting structured patient/caregiver interviews, reviewing the documented external prescription history, and contacting outpatient pharmacies to obtain accurate medication histories. This shall happen once the patient is admitted or prior to admission, as directed.

The MHT will assist in the provision of pharmaceutical services under the direct supervision of the Registered Pharmacist by performing the technical and clerical duties involved in all medications, pharmaceutical products, and services for all adult patients. Work and/or performance shall conform to written established hospital policies and procedures.

Qualifications:

Required:

  • Graduate of an accredited training program or two years of experience.
  • Registered by the New Jersey Board of Pharmacy.
  • Required PTCB Certification and obtaining of Medication History Certification within 1 year of employment.

  • Full-Time
  • Evenings

Essential Functions:

  • Access the Electronic Health Record (EHR) in the Emergency Department to review outpatient medications and identify patients with opportunities to optimize therapy.
  • Verify the collected information using external reliable sources, such as outpatient pharmacies, to resolve any medication concerns.
  • Update and reconcile the patient's outpatient medication list in the EHR for provider and pharmacist review.
  • Demonstrate knowledge of the institution's formulary and notify appropriate parties for non-formulary medications on a patient's outpatient medication list.
  • Document patient care activities in the form of interventions, according to the organization's policies and procedures.
  • Follow all procedures involved in the appropriate documentation of medication errors and meet the minimum required documentation standards per employment status.
  • Inspect floor stock storage locations for expired medication and remove/replace outdated pharmacy products.
  • Make every attempt to be knowledgeable of changing policies and procedures.

  • Maintains the confidentiality, security, and integrity for the management of associate/patient information according to policy and procedure.
  • Participates in ongoing education and training related to organization and department-specific requirements, including, but not limited to, safety and infection control management programs.
  • Performs other duties and responsibilities as determined by the pharmacist on duty or other supervisory personnel.
  • Promptly answers telephones using proper departmental etiquette.
  • Remains pleasant and courteous in daily interactions with others, especially under adverse conditions.
  • Provides a compassionate and supportive environment with prompt and consistent service.
  • Possesses strong communication skills to interview patients and caregivers, and well as interact with the interdisciplinary team.
  • Clear understanding of working with an interdisciplinary team is strongly desired.
  • Good working knowledge of healthcare informatics and EHR use is required for the documentation of daily activities and follow-up to providers conducting final medication reconciliation steps.
