Responsible for the development and implementation of services/systems toward an organized, multidisciplinary approach to the delivery of care to patients and program goals. Responsible for promoting the continuity of care for patients through ongoing education, development of programs, evaluation of outcomes, and direct interaction with patients, families, and groups.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Associate's degree in Nursing.

2. Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).

3. Obtain certification in Basic Life Support within 30 days of hire.

EXPERIENCE:

1. Three (3) years of clinical experience in nursing.

PREFERREDQUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Bachelor's degree in Nursing (BSN).

2. Certification in area of specialty.

EXPERIENCE:

1. Five (5) years of clinical experience in Nursing.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. 1. Directs and oversees Performance Improvement initiatives.

2. Collaborates with physicians, nursing staff, and other health care providers to ensure comprehensive, coordinated patient and family education.

3. Identifies opportunities for improvement in patient outcomes and participates in plans to enhance clinical outcomes and patient satisfaction.

4. Coordinates and oversees preparation and submission of education programs to meet national standards of accreditation for patients, nurses, exercise physiologists and exercise specialists.

5. Prepares and performs annual review of the program.

6. Assists in the revision and review of policies and protocols.

7. Assists with marketing and program preparation.

8. Conducts daily rounds and/or audits on applicable patient population, as needed.

9. Supervises, trains, and promotes the development of staff and provides direction as needed.
