JOB DESCRIPTION:

SUMMARY OF POSITION

The CCBHC Linkage and Engagement Provider is primarily responsible for connecting clients, families, and community partners to essential resources. This role includes providing emergency mental health consultations and ensuring follow-up appointments are made when clinically appropriate. The position requires active participation in cross-functional teams and performance improvement initiatives, with a focus on making informed decisions that enhance client care.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Provide linkage services to clients, families, community partners and referral sources.
  • Complete all required documentation and comply with contracts, grants, standards, regulations, and laws.
  • Attend staff meetings and maintain appropriate communication with other employees.
  • Provide follow-up appointments, Brief Therapy as clinically appropriate, and complete IATP as needed to link individuals with Centerstone services.
  • Offer emergency mental health consultations at office locations.
  • Participate in training to enhance job-related skills.
  • Deliver community education as assigned.

KNOWLEDGE, SKILLS & ABILITIES

  • Knowledge of effective communication methods including written, verbal, in-person, and virtual interactions.
  • Proficiency in customer service techniques, demonstrated through successful resolution of client inquiries.
  • Experience in clinical assessment for mental health and substance use disorders, with examples of diagnostic evaluations.
  • Ability to develop and implement problem-solving strategies, illustrated by case studies or specific scenarios.
  • Familiarity with electronic health record systems and their application in clinical settings.
  • Understanding of mental health regulations and compliance standards relevant to practice.
  • Knowledge of community resources and referral processes for clients in need of additional support.

QUALIFICATIONS

Education Level

Bachelor's Degree in Human Services field, or Bachelor's Degree with MHP Status.

Years of Experience

Experience working with the public or a demonstrated interest to work with different population.

Certification/Licensure

Position requires valid driver's license and safe driving record. CPR certification within 90 days of hire.

$19.10--$26.80
