Job Summary

In conjunction with the Sub-Acute Manager plans, organizes and coordinates resident care on Sub-Acute Unit ensuring quality care is provided. Responsible documentation reviews and audits to promote continuous quality improvement. Ensures nursing practice is consistent with scope, regulatory requirements and policies and procedures. Coordinates nursing care and fosters cooperation between staff, shifts and departments. This position assists the charge nurse with daily operation of the respective units.

Essential Functions
  • Provides broad and expansive yet intricate and individualized clinical review and recommendations resulting in enhanced clinical and competencies and compliance.
  • Conducts thorough chart reviews; works with MDS Nurse and Program Nurse Manager to assure that follow through on plans of care are appropriately documented.
  • Reviews Physician Orders and Skilled Charting for thoroughness and follow through.
  • Performs documentation audits as assigned.
  • Attends Medicare "At-Risk" meetings with collaboration with IDT.
  • Identifies and participates in the preparation of the POC of any Survey and/ or Past Non Compliance as necessary. Performs POC audits as directed.
  • Assists with Policy and Procedure review, revision and development as needed.
  • Provides added clinical and managerial support where needed for improved quality of care for compliance and accountability.
  • Makes daily rounds assuring quality of care has been delivered. Checks 24 hour reports, reviews medical record of all new admissions for completed assessments, care plans, nurse's note, and physician's orders. Greets new admissions with inquires within the first 24 hour stay.
  • Uses clinical assessment skills to observe residents functional levels and changing conditions.
  • Receives reports from unit charge nurses and works closely with them to ensure quality care is delivered.
  • Communicates with resident's family members to assess needs and concerns as needed, answering questions and lending support.
  • Educates resident and families in self-care practices as needed to reach the highest functional levels, and prepares the resident for discharge.
  • Observes residents for functional levels and changing conditions.
  • Performs daily review of all unit reports and works with charge nurses to ensure quality care is delivered.
  • Assists with new and re-admissions assessments.
  • Attends Care Conference as scheduled.
  • Assists medical professionals in providing services to all residents.
  • Ensures Medicare documentation reflects skilled services are being provided.
  • Performs other duties as assigned.
Qualifications

Required

  • Associate's degree, Graduate of an accredited school of nursing with an associate's degree or higher.
  • 3 years of relevant experience Minimum of three years' experience as a nurse with ability to demonstrate an understanding of the nursing process
  • 1 year of relevant experience, nursing skills with a minimum of one year of supervisory experience
  • LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan Upon Hire
  • CRT-Basic Life Support (BLS) - AHA American Heart Association Or CRT-Basic Life Support (BLS) - ARC American Red Cross
