Full-time Day/Night (5x7.5 shifts). Must hold a NYS Nursing Assistant Certification and BLS (AHA or RQI only). Requires one year of previous experience in long-term care (LTC) and must be licensed for at least one year.

Education
Must have a High School Diploma or GED.
Job Functions
- Ensure resident's Activities of Daily Living (ADLs) needs are met as outlined in the plan of care and in accordance with policies and procedures within a safe and comfortable environment. This includes hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, skin care, etc. Documentation must be completed per protocol.
- Maintain resident's environment and safety as outlined in the plan of care and in accordance with policies and procedures. This includes answering call lights and alarms promptly, providing safety mats and low beds, using restraints under the direction of Nursing, maintaining cleanliness of the unit, behavior and fall prevention, appropriate use and monitoring of malfunctioning equipment, etc. Documentation must be completed per protocol.
- Meet resident's nutritional needs as outlined in the plan of care and in accordance with policies and procedures. This includes meal delivery, tray preparation, adaptive equipment, feeding, and nourishments. Resident's preferences and choices shall be honored. Documentation must be completed per protocol.
- Meet resident's rehabilitative (restorative and maintenance) needs as outlined in the plan of care and in accordance with policies and procedures. This includes range of motion, ambulation, transfers, proper body alignment, and use of assistive devices. Documentation must be completed per protocol.
- Report resident's rights, refusals, and concerns to the Charge and/or Supervising Nurse in a timely fashion in accordance with policies and procedures. Identify and report changes in resident physical, emotional, and psychosocial status to the Charge Nurse promptly.
Occupational exposure to blood borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
