Job Title: Clinical Pharmacy Manager - Ambulatory Transplant Department: Pharmacy Services Position Summary: The Ambulatory Clinical Pharmacy Manager in Solid Organ Transplantation is to lead clinical pharmacy services across outpatient transplant programs. This role will be responsible for overseeing the design, implementation, and maintenance of Collaborative Drug Therapy Management (CDTM) protocols and providing direct patient care in transplant. The pharmacist will collaborate closely with transplant providers and pharmacists to optimize therapy and improve patient outcomes. Key Responsibilities: Program Leadership
- Provide strategic leadership and oversight for the Solid Organ Transplant Clinical Pharmacy Program, ensuring alignment with institutional goals, transplant program priorities, and regulatory requirements (CMS, and Joint Commission, etc.).
- Lead the development, implementation, and evaluation of clinical pharmacy services for kidney, liver, heart, and/or lung transplant programs.
- Develop and maintain transplant-related clinical practice guidelines, protocols, and collaborative drug therapy management (CDTM) agreements.
Direct Patient Care Responsibilities
- Provide comprehensive pharmacotherapeutic management for pre- and post-transplant patients, including immunosuppression, infection prophylaxis, and management of comorbidities.
- Ensure safe and effective medication use through therapeutic drug monitoring, pharmacokinetic consultation, and optimization of individualized treatment plans.
- Conduct medication reconciliation, patient education, and discharge counseling for transplant recipients and living donors.
- Support transitions of care between inpatient and outpatient transplant settings to reduce medication errors and hospital readmissions.
Collaboration and Education
- Collaborate with physicians and care coordinators to integrate pharmacy services into care pathways.
- Serve as a preceptor for pharmacy residents and APPE (Advanced Pharmacy Practice Experience) students, providing clinical instruction, mentorship, and evaluation during transplant ambulatory rotation.
Regulatory Compliance and Reporting
- Participate in transplant ambulatory quality initiatives
- Ensure adherence to CDTM regulatory requirements, proper documentation, and risk mitigation strategies.
Research & Performance Improvement
- Participate in or lead clinical research projects and quality improvement initiatives related to transplant pharmacotherapy, immunosuppression management, or medication adherence.
- Evaluate and present outcomes of transplant pharmacy interventions, including cost-effectiveness and patient safety metrics.
- Contribute to data collection and analysis for transplant program performance indicators and pharmacy service benchmarks.
- Support pharmacy residents and students in research projects, publications, and presentations related to cardiovascular pharmacotherapy.
- Maintain awareness of emerging literature, guidelines, and best practices to inform program development and evidence-based interventions.
Eligibility Requirements: Education & Licensure
- Doctor of Pharmacy from an ACPE-accredited college of pharmacy.
- Valid NYS Pharmacist License and Registration.
- Completion of PGY2 Residency in Solid Organ Transplant highly preferred.
- Board certification in Solid Organ Transplant (BCTXP) preferred.
Experience
- Prior leadership or supervisory experience preferred.
- Experience in academic or teaching hospitals is desirable.
- Experience in research/performance improvement is desirable.
Knowledge & Skills
- In-depth knowledge of transplant pharmacotherapy.
- Excellent analytical, organizational, communication, and teaching skills.
- Strong commitment to patient safety, evidence-based practice, and quality improvement.
