Description
The Clinical Pharmacy Liaison is integral to supporting the clinical team in ensuring the safe, effective, and timely administration of medication for patients managing diabetes and associated conditions. Working under the supervision of the Director of Specialty Pharmacy Services and in collaboration with diabetes educators, providers, and care teams, the liaison plays a critical role in assisting patients with insulin, oral diabetes medications, continuous glucose monitors (CGMs), insulin pumps, and other diabetes-related therapies, ensuring patients have timely access to the medications and supplies they need as part of their hospital-based diabetes care plan. This position demands a strong attention to detail, proficiency in diabetes medications and supplies, and a patient-centered approach to healthcare.
Qualifications
Minimum Education
High School Diploma or GED (Required) Minimum Work Experience 2 years Relevant pharmacy work experience (Required) Required Skills/Knowledge Knowledge of diabetes medications, devices, and current standards of care. Familiarity with insurance billing, prior authorization processes, and patient assistance programs Excellent organizational, communication, and teamwork skills. Strong patient-centered focus with the ability to work in a fast-paced hospital environment. Excellent verbal and written English communication skills required; as well as ability to communicate professionally over the phone. Excellent interpersonal skills to provide superb, personalized customer service to instill confidence and advocate for patients; ability to explain required information to customers in a comprehensible manner Cultural sensitivity, understanding, and comfort with a wide range of social, racial, and ethnic populations Flexibility to adapt to changes in the departmental needs (i.e., assisting other team members, adjusting assignments, etc.). Ability to quickly learn other relevant applications that support management of patient care and assigned responsibilities; and ability to extract necessary information.
