Job Summary and Responsibilities

Job Summary

Join Acentra Health as a PASRR Evaluator in Wilkesboro, North Carolina. In this role, you will conduct PASRR Level II Pre-Admission Screening and Resident Review assessments to determine appropriate placements and next levels of care for individuals with complex needs, helping to support improved health outcomes.

As a PASRR Evaluator, you will conduct assessments for geriatric individuals and those living with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) in nursing homes, hospitals, and community-based settings in Wilkes, Caldwell & Catawba Counties. This PRN hybrid role offers the flexibility of remote work from your home office with some local travel, allowing you to make a meaningful impact within your community. We also reimburse travel-related expenses, including mileage and cover parking.We provide all essential technology and equipment for your home office, including a company-issued laptop for both remote and field work.

*This position is hybrid. Candidates must be comfortable commuting to any of the following North Carolina counties; Wilkes, Caldwell, or Catawba to support the fieldwork required for the role.*

Job Responsibilities:

  • Conduct comprehensive clinical assessments by reviewing patient history, medical records, and collateral information to ensure accurate evaluations and recommendations.
  • Recommend appropriate levels of care and service needs based on the individual's physical, behavioral, and psychological functioning.
  • Apply strong clinical knowledge of psychiatric disorders as defined in the most current Diagnostic and Statistical Manual of Mental Disorders (DSM) used in North Carolina.
  • Perform person-centered assessments and provide meaningful, evidence-based recommendations for appropriate levels of care.
  • Interact with healthcare professionals, patients, their families, and internal partners as needed to support the assessment and review process.
  • Complete assessments in a timely manner and within scope of practice to meet all contractual and program requirements.
  • Leverage technology, computer systems, web-based applications, and proprietary software platforms to efficiently perform job functions and manage and track assessment activitie
  • Maintain a working knowledge of applicable federal regulations, including 42 CFR, Chapter 483, and ensure compliance in all assessment activities.
  • Understand and adhere to all corporate policies, including those related to HIPAA and its Privacy and Security Rules.

Qualifications

Required Qualifications and Experience

  • Active, unrestricted licensure in the state of North Carolina in a healthcare discipline such as Licensed Clinical Psychologist (LCP), Licensed Clinical Social Worker (LCSW), Licensed Clinical Mental Health Counselor (LCMHC), Licensed Psychological Associate (LPA), or Registered Nurse (RN) with a master's degree in Psychiatric Nursing or a closely related field.
  • Graduation from an accredited college with a relevant degree program and a minimum 2 - 3 years of clinical experience providing services to individuals with mental illness.
  • Minimum 1 year of experience working with individuals with intellectual or developmental disabilities or related conditions.

  • Experience in medical record abstraction and familiarity with medical record systems, medical terminology, and disease processes.

Preferred Qualifications and Experience

  • Ability to assess psychological, emotional, behavioral, and physical functioning to support accurate clinical evaluations.
  • Knowledge of standards of practice related to mental health, intellectual and developmental disabilities, geriatrics, and other disorders typically requiring long-term care services and support.
  • Previous experience working with the geriatric population is preferred.
  • Strong attention to details, with the ability to effectively prioritize, manage, and complete tasks in a timely manner.
  • Proficiency in Microsoft Office Suite, including Excel, Word, PowerPoint, Outlook, and Teams, as well as experience using other web-based tools and platforms.

Compensation

The compensation for this role is $37.00 per hour

Based on our compensation program, an applicant's position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
