DESCRIPTION

Works directly with physician lead and administrative leadership to coordinate program and provide program specific services. The role of the Gender Health Program Coordinator is dedicated to ensuring that transgender patients are able to successfully navigate all UCSDH resources available to them. Will address patient concerns, inform them of available services and help navigate insurance plans. In addition, the navigator interfaces with hospital and clinical leadership to ensure that the full spectrum of patient needs and services are addressed and delivered seamlessly. Including but not limited to the following:

  • Health promotion
  • Patient concerns
  • Advocacy and outreach
  • Disease prevention
  • Innovations
  • Counseling referrals
  • Community referrals
Specialized Scope of Service:
  • LGBTQIA+ Community

  • Group Facilitation Experience

  • Clinical assessment knowledge

  • Letters of readiness for surgery

MINIMUM QUALIFICATIONS
  • A Master's Degree from an accredited school of social work or equivalent.

  • Three (3) years of relevant experience.

  • Experience and proven success serving as a clinical resource and liaison for patients and their families and assisting with navigating all aspects of the healthcare system.

  • Clinical background with experience providing clinical education, and serving as a liaison with clinical service providers is required.

  • Good communication and interpersonal skills and ability to speak concisely to clients, navigators, and interact with other care team members.

  • Strong knowledge of Gender affirming care, case management, and patient navigation.

  • Organized with confidential materials, caseloads, appointment tracking.

  • Ability to build relationships with different types of people including clients, organizational members and members of the interdisciplinary care team.

PREFERRED QUALIFICATIONS
  • Licensed Clinical Social Worker (LCSW) - issued by the state of California.

  • Bilingual English/Spanish.

  • Ability to create educational content related to best practices in gender health care

SPECIAL CONDITIONS
  • Must be willing to work various hours and locations based on business needs.
  • Position will float between Hillcrest and La Jolla location.
  • Employment is subject to a criminal background check and pre-employment physical.
