Responsibilities The Exercise Physiologist-Diabetes serves as a provider of physical activity education and fitness evaluation and assessment in diabetes and endocrinology within the Norton Healthcare System; provides direct patient care and support, therapeutic intervention, including evaluation and assessment of physical fitness and health and its relationship to chronic disease management. The Exercise Physiologist-Diabetes manages and evaluates the patient's diabetes care plan and activity needs; performs fitness evaluations and develop tailored exercise programs for patients with diabetes to improve their physical fitness and well-being; addresses individual patient needs, while assessing comprehension, psychosocial factors, and social determinants of health, and supporting coordination of resources. This role functions as part of a multidisciplinary team utilizing age-appropriate principles of growth and development for patients aged infant to geriatric, participates as needed with organizational departments/committees and partner with programs to provide robust health and wellness options to patients served, and support continued growth and improvement of continuum of care throughout their department and organization. The Exercise Physiologist-Diabetes administers comprehensive physical fitness and health services and relevant health and wellness education to patients and their families in the assigned settings via face-to-face or remote electronic modalities.
