This role will be full-time, Monday - Friday; regular business hours: 8:30 AM - 5:00 PM; no weekends, no holidays. Registered Nurse (RN) Outpatient Palliative Care Job Responsibilities:
- Performs essential duties necessary to clinic or non-acute operations to include taking vitals, stocking exam rooms, giving vaccines, blood draws, etc.
- Formulates plans of care based on trends of similar patients including patient education and discharge planning. Identifies variances from expected outcomes. Evaluates patient outcomes and makes revisions to plans of care.
- Collaborates with patients/families to identify realistic desired outcomes. Adapts standards and guidelines for individual patients. Tailors and prioritizes caring practice to individual needs including cultural, ethical and spiritual needs. Participates in the planning of routine transitional healthcare needs (i.e. treatment options, patient placement options, end of life options). Responds to changes in patients' needs and provides holistic caring.
- Makes independent clinical decisions based on knowledge drawn from education and experience.
- Actively advocates for patients' rights and identifies potential conflicts.
- Ensures efficiency in the delivery of care. Determines immediate priorities and offers solutions for problems. Works with the team to achieve maximum productivity.

- Documents all patient care activities as per documentation standards. Assists the healthcare team with the documentation process.
- Participates in work groups and/or committees. Keeps informed of initiatives and incorporates the outcomes of team or committee work into practice.
- Acts as a clinical resource and serves as a mentor to healthcare team members while displaying leadership behaviors.

Minimum Qualifications:
- Education: Bachelor's Degree Nursing or Associate's Degree. If RN has an Associate's Degree, within 2 years of date of hire, they must meet with their nurse leader and conduct the following: 1.) Identify which accredited school they plan to attend 2.) Provide a written plan with anticipated BSN completion date 3.) Submit a review of transcripts from the school indicating the required pre-requisites and timeline for taking the courses 4.) Complete BSN within 5 years of start date.

- Certification: Basic Life Support from the American Heart Association; Licensed as a Registered Nurse in the state of employment or an active multi-state Registered Nurse license
Preferred Qualifications:
- Experience: Experienced nurse due to the delicate population. Prior palliative care/Heart Failure/Oncology experience as a RN.
