Job Profile Summary

This role focuses on developing and implementing programs to establish, maintain, and improve patient quality care standards. In addition, this role focuses on performing the following Performance Improvement/Quality duties: Identifies and executes performance improvement and quality opportunities across the enterprise, enabling successful transformations and driving cost savings, process and product quality, and achievement of business goals. Responsibilities also include partnering with business leaders to provide expert insight on existing processes and procedures, applies process improvement methodologies to achieve PI/Quality objectives, and builds process improvement capabilities. An organizational related support or service (administrative or clerical) role or a role that focuses on support of daily business activities (e.g., technical, clinical, non-clinical) operating in a "hands on" environment. The majority of time is spent in the delivery of support services or activities, typically under supervision. An experienced level role that requires basic knowledge of job procedures and tools obtained through work experience and may require vocational or technical education. Works under moderate supervision, problems are typically of a routine nature, but may at times require interpretation or deviation from standard procedures, and communicates information that requires some explanation or interpretation.

Job Overview

This position is responsible for review of and for outreach to assigned patient populations to support assigned primary care practices in achieving the ongoing quality goals for all patients. Assist in design and application of increasingly creative patient outreach techniques and tracking all activity for reporting to management. Work collaboratively with primary care practices leadership. Use various patient data systems, including Epic, and Arcadia share file in performance of duties and responsibilities.

Job Description

Minimum Qualifications:

1. High school diploma or equivalent.

2. One (1) year of experience in public health, social work, or clinical field.

Preferred Qualifications:

1. Associates degree in public health, social work, or clinical field.

2. Two (2) years of experience in public health, social work, or clinical field.

Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

1. Review and implement programs to each defined matrix and maintain patient outreach thresholds.

2. Ensure highest level of achievement by engaging patients to schedule and keep appointments for their recommended exams and screenings.

3. Create and maintain patient information electronic health record related to the various recommended exams and screenings, to ensure that patients receive and understand the rationale for the recommendations. Work with management to ensure patients follow through and arrive for scheduled exam.

4. Create and maintain weekly reports as required.

5. Analyze outreach progress and adjust EPIC patient flags as indicated to communicate to providers.

6. Create and maintain individual physician patient rosters and communicate progress on measures to the Quality Team and Practice Managers.

7. Provide notification to physicians regarding screenings patients have had at outside labs or facilities, and ensure clinical documentation is added to the electronic medical record (EMR) and added to assessment.

8. Contact patients interested in establishing care with a provider. Help patients identify the optimal provider based on their needs and provider availability and schedule the initial appointment and any recommended tests or screenings. Explain any paperwork and documentation needed for initial visit.

9. Review data reports to track completion of health care screenings and tests for internal review as well as patient outreach.

10. Utilize multiple methods of patient contact, consistent with project and patient preference. Collaborate between Specialists and patients to ensure patients' needs are met through various methods of contact.

11. Work on special projects and other job-related tasks as required.

Physical Requirements:

1. Frequent sitting, occasional standing or working, and lifting of 10-15 lbs.

2. Manual dexterity using fine hand manipulations for computer keyboard operation.

3. Requires ability to see computer screen and read reports.

4. Requires ability to hear instructions.
