SUMMARY:

The Community Resource and Substance Use Specialist supports integrated behavioral health within primary care by coordinating care for patients with behavioral health, substance use disorder (SUD), and social needs. This role combines resource navigation with substance use counseling and education to improve patient outcomes and access to care.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Responds to patient and provider requests for assistance with substance use concerns to facilitate connection to treatment.

Triages patients identified with social determinants of health (SDOH), behavioral health, or substance use disorder (SUD) needs to coordinate and track referrals to internal or external resources.

Coordinates behavioral health referrals to internal and external behavioral health clinicians and resources, including referrals to higher levels of care where indicated.

Conducts screenings, assessments and intakes for patients referred by primary care providers for substance abuse counseling.

Educates patients, families, providers and care team members about substance use disorder treatment options and recovery resources.

Develops and implements individualized treatment plans using evidence-based practices.

Collaborates with primary care providers and other care team members to ensure coordinated care.

Maintains extensive knowledge of community resources and eligibility guidelines for each.

Complies with federal and local confidentiality laws, including HIPAA, ensuring patient privacy.

Performs other duties as assigned.

EDUCATION:

Bachelor's degree in a related field

Licensed Chemical Dependency Professional (LCDP) in Rhode Island

EXPERIENCE:

Minimum of 2 years of experience in substance use counseling

Additional training in the medical field with a social service background or demonstrated possession of the competencies necessary to perform the work

Experience working in a clinical setting with a focus on behavioral health or social services

Proficiency in the use of an EMR and knowledge of medical terminology

Ability to handle high volume and diverse assignments

Experience with motivational interviewing, CBT and other therapeutic modalities preferred

Exercise sound judgment and decision-making. Able to assess and differentiate priorities

Excellent interpersonal skills and ability to work with and through others to achieve results

Excellent written and verbal communication skills

Must handle the most sensitive and confidential matters with the utmost discretion

Proficiency in use of various applications such as Microsoft Word, Excel, Web-based applications and EMR/EHR

Pay Range:

$54,637.86-$90,148.24
