SUMMARY:
The IntegratedBehavioral Health (IBH) Clinicianwillworkclosely with our primary care teams to provide quick access to behavioralhealth assessment, triage, and brief treatment. The IBH clinician is embedded in the primary care practice and works directly with care teams to assess patients with behavioral health and/or substance use concerns. The scope of service includes but is not limited to brief assessment, crisis management, triage, as well as mental health/substance abuse treatment within the scope of their clinical abilities and training.

RESPONSIBILITIES:
Collaborates with primary care providers in recognizing, treating and managing mental health and psychosocial issues and acts as a contributing member to the primary care team.
Triages both scheduled and unscheduled referrals from the medical team, treating some with brief treatmentntervention and referring others to more appropriate resources.
Provides brief treatment interventions for mild to moderate behavioral health concerns as well as crisis intervention.
Consults in real time to assist patients and families with identified psychosocial issues and determining the appropriate level of care for patients.
Conducts patient care in person, telephonically or via telemedicine for consultations and psychological treatment.
Develops, when indicated, relapse prevention plans and helps patients maintain stable functioning.
Assists in the identification of "at risk" patients and in the development of plans to prevent escalation of existing conditions.
Assists patients with self-management through education and telephonic/telemedicine engagement.
Assists patients in navigating the healthcare system and serves as a liaison to external behavioral health care, including schools and other health care services outside of Brown Health Medical Group Primary Care.
Assesses the health care, educational and psychosocial needs of the patient/family.
Participates in interdisciplinary care conferences with a focus on behavioral and social needs.
Collaborates with the PCP, patient, members of the healthcare team including the office care manager and care navigators, as well as required healthcare services outside of Brown Health Medical Group Primary Care
Ensures open communication regarding patient status with physicians, office staff and patients' families.
Attends required trainings and practice team meetings as scheduled.
Performs other job-related duties as required.
Provide behavioral health education and training to staff as needed
MINIMUM QUALIFICATIONS:
Education and Experience
Licensed Independent Social Worker (LICSW), Licensed Clinical Psychologist, or Licensed Mental Health Counselor (LMHC) licensed in the State of Rhode Island and in good standing
Prior experience working in a behavioral health or medical setting with child clinical training
Skills and Abilities
Ability to provide services, telephonically and through telemedicine
Ability to participate in ongoing quality assurance and improvement processes
Ability to communicate compassion while maintaining equanimity
Ability to create systems and improvement processes as well as adapt to changing care processes
Excellent interpersonal skills and ability to work with and through others to achieve results
Ability to maintain confidentiality with all aspects of information (including patient data) in accordance with the practice's philosophy and policies as well as state and federal regulations
Ability to work collaboratively with a wide range of internal and external stakeholders
Superior organization and prioritization skills
Excellent written and verbal communication skills
Advanced proficiency with computer programs and other technology (i.e., Medical Record, Microsoft Word, Excel and Access, and Web-based applications)
Maintaining familiarity with culturally appropriate resources and skills in cultural competency
Possess a working knowledge of the biopsychosocial issues impacting an individual with a chronic medical problem
Possess strong work ethic and dedication to the well-being of patients, families and the practice

This role works primarily in an office setting with some scheduled hybrid work time.
Sit and stand for prolonged periods of time

Manual dexterity to operate personal computers and standard office machines and equipment as it pertains to one's position
Visual Acuity, Hearing and Speaking
To transfer information from paper to computer, from computer to computer, and to communicate in person and on the telephone

Pay Range:

$60,211.42-$99,331.96
