Job Description

Elwyn seeks a Registered Nurse House Manager who will be the lead staff on the PM shift, under the direction of the Program Administrator, for one 962 residential home. Responsible for ensuring that nursing care and directives are carried out in the home as directed by the 962 Program Administrator. Responsible for ensuring that any suspicion of or knowledge of suspected abuse is reported in accordance with the law and program policies. "Mandated Reporter" (SOC 341).
DUTIES AND RESPONSIBILITIES:
  • Performs all nursing procedures within RN scope of practice, including head-to-toe assessment.
  • Acts independently to collect and assess vital signs and data in order to notify physician and Interdisciplinary Team (IDTeam) members of resident's condition in a timely manner.
  • Develops, implements, maintains, and updates Health Care Plans and IHCPs for all residents
  • Ensures that all medical documentation in the home are completed, including but not limited to: nursing notes, weekly summaries, 24 Hour Log, Medication Administration Record, and Centrally Stored Medication Log.
  • Ensures that the Physician's Orders are being followed as written
  • Documents and reports all events as required by DDS, Regional Center, and CCL
  • Responsible for training staff on clinical aspects of health care and Health Care Plans.
  • Ensures the proper level of equipment and supplies are maintained
  • Ensures proper documentation is maintained
  • Respects at all times the confidentiality of residential records and uses complete discretion when discussing information.
  • Maintains communication with Program Administrator, families, hospitals, etc.
  • Supports residents with self-care, self-administration of medications, and independent living skills including but not limited to bathing, dressing, feeding, and grooming. Supports residents in activities of daily living, personal hygiene, and household activities including but not limited to grocery shopping, food preparation, serving and clean-up, as indicated in the IPP.
  • Implements Behavior Plans if necessary.
  • Member of the IDTeam in designated aspects of the development and implementation of the Individual Service Plan (ISP) and Individual Program Plan (IPP) outcomes, and overall programming for residents.
  • Supports residents in occupational, recreational, vocational, and therapeutic programs.
  • Maintains a safe, sanitary, and therapeutic environment which promotes respect and dignity for residents and protects the privacy, rights, confidentiality, and physical/emotional well-being of residents.
  • Engages residents in activities that provide opportunities for learning and practice of skills tailored to the specific needs of the resident.
  • Supports residents in the constructive use of leisure time. Encourages residents to participate in planned recreational activities.
  • Actively supervises during dining, recreational activities, medical and other appointments. Knows and reports the whereabouts of the assigned residents at all times.
  • May transport residents in the company vehicle to medical, recreational and/or other appointments.
  • Attends staff meetings; including and not limited to in-services and training sessions.
  • Maintains all required trainings and certifications as required by regulations and Elwyn CA.
  • Reports and documents medical/behavioral concerns and other special incidents to proper supervisory personnel.
  • Remains on duty until endorsement is completed with incoming licensed staff.
  • Consistently demonstrates the ability to maintain quality assurance standards and promotes Elwyn CA goals and objectives in accordance with the organization's policies and procedures.
  • Continually works with staff members in creating a professional and amiable atmosphere; encourages and supports co-workers to work as a team.
  • Provides ongoing training and feedback to DSPs, LPTs, and LVNs as needed.
  • Demonstrates the ability to adapt to changes in workload.
  • Assists with health care and/or behavioral needs and documents appropriately.
  • Completes assigned work in a timely manner and strives to make good use of time.
  • Gives feedback to Program Administrator regarding any issues affecting home operations and resident care; including immediate notification of any changes in care needs.
  • Collaborates with Program Administrator regarding staffing needs.
  • Seeks and accepts help and/or guidance from Program Administrator, Director of Nursing, and Elwyn CA Administration as appropriate.
  • Responds to emergencies appropriately as directed by all health care/behavior plans, regulations, and/or policies and procedures.
  • Performs other duties as assigned.

EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:

  • Active Professional Nurse (RN) Licensure in good standing in the State of California.
  • Thirty continuing education credits every two years to maintain licensure or as required to meet the minimum standards of California licensure.
  • Bachelor's degree preferred.
  • Current First Aid/CPR certification.
  • Must possess a current, valid driver's license in state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record
  • Prefer one year experience in a residential setting for persons with developmental disabilities. Minimum one year recent medical surgical experience preferred.
  • Must hold valid or else complete a DOJ and FBI fingerprint and criminal clearance.
  • Must have a current, within one year of hire or within 7 days after hire, a health screening including a TB test.
  • Previous supervisory experience preferred including strong interpersonal, communication, leadership skills.
  • Computer skills required.
