Job Title: Case Worker / Social Worker Exp with Medically Assisted Treatment Location: Frackville, PA 17931 Duration: Contract Job Mode: On-Site Pay rate: $39.18/hour on w2 Shift hours: Monday through Friday from 8:00am - 4:00pm Job Description: The Candidate shall communicate with SCI when reentrants OD in community and are returned to SCI for detox to determine eligibility and appropriateness for . The SCI Candidate shall assist with referrals in the community, including setting up and scheduling appointments for medication and treatment. The SCI Candidate shall ensure needs, benefits, and services are obtained in accordance with assigned individuals' treatment plans. The SCI Candidate shall track and update spreadsheet/database on everyone released from an SCI on a . The SCI Candidate shall attend weekly Vivitrol Conference Call, and other meetings as needed. Candidates shall document all contacts in appropriate systems (Captor, Sapphire etc.). Candidates shall attend all required trainings. Candidates shall meet with reentrants with positive urine drug screens to assess and refer to treatment. Candidates shall complete the GPRA as required (Initial, 6 months, discharge). Candidates shall meet with Reentrants in the SCI, , Parole Offices, and/or Treatment providers as necessary. Candidates shall communicate all treatment planning to Center Staff, SCI Staff, and/or Parole staff. Candidates shall subscribe to the /AOD Treatment framework and policies and procedures of the DOC. Candidates shall implement and provide social work, counseling, and case management services to individuals to enhance their social functioning and to help them attain a more satisfactory social, economic, or physical adjustment. Candidates shall perform in accordance with applicable rules, regulations, policies, and procedures under the general supervision of an administrative supervisor. Candidates shall assess offenders' psychosocial strengths, weaknesses, and social work service's needs, and eligibility for services. Candidates shall counsel individuals utilizing various treatment modalities. Candidates shall discuss assigned individuals' psychosocial assessments and family history in multidisciplinary treatment team meetings. A Bachelor's in Social Work or Human Service field required. Required Skills: Knowledge of professional social work principles, practices, and methods. Knowledge of professional casework principles, practices, and methods. Knowledge of individual and group behavior. Knowledge of counseling techniques and interventions used to recognize or improve psychosocial, economic, and physiological situations of individuals and families. Knowledge of commonly occurring physical and mental illnesses and their social, economic, and psychological implications. Knowledge of social, economic, health, habilitative, and rehabilitative problems, treatment methods, and resources. Knowledge of the requirements of the Health Insurance Portability and Accountability Act (HIPAA) as well as state confidentiality laws as applied to Social Work practice. Ability to read and interpret rules, regulations, policies, and procedures relating to SoCal work services and programs. Proficient in Microsoft Excel.
