Summary
Restores Patient Physical Functioning by interpreting physical therapy evaluations and test results, determining physical therapy treatment goals and plans, administering manual exercises, and instructing, encouraging, and assisting patients in performing physical activities and using assistive and supportive devices. They will be responsible for Physical Therapy Students and Physical Therapy Assistants, as well as training new staff as needed. They will also participate in program development and research as required per site.

Essential Functions
-Establishes therapy goals and formulates a treatment plan based on evaluation results.
-Supervises PTAs in accordance with organizational policy and state regulations.
-Provides individual, family, and group therapy sessions for clients engaged in outpatient therapy
-Participates in interdisciplinary groups for hospice patients and case conference meetings for home health patients.
-Participates in discharge planning.
-Provide initial assessment of clinical needs using standardized tools and protocols, and facilitate recommendations for treatment.
-Record all pertinent data in the patient's medical records.

Education
Bachelor's Degree Physical Therapy required or Master's Degree Physical Therapy required or Doctor of Physical Therapy Physical Therapy preferred

Experience
Experience in a hospital, rehab or ambulatory setting 2-3 years required
