Pharmacy Resident PGY1

SUMMARY:

Reports to Residency Program Director, Pharmacy Services. Under general supervision of the Pharmacist-in-charge or a Pharmacy Supervisor, interprets, evaluates, prepares, and dispenses medications pursuant to a legitimate order or prescription in keeping with established policies, procedures, standards of practice, and scope of pharmacy services. Collaborates with multidisciplinary care providers to monitor, assess, and intervene as needed to ensure safe, appropriate, and cost-effective medication use. Oversees and closely reviews the work of pharmacy technicians and interns as applicable.

RESPONSIBILITIES:

Provides safe and effective patient care to a diverse range of patients, including those with multiple co-morbidities, high-risk medications, and multiple medication regimens.
Applies pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective use of medications for the patient populations served.
Determines therapeutic appropriateness of the patient's drug regimen as it relates to the patient's clinical status, concomitant drug therapy and diet, and with consideration of the aging processes and human developmental stages (neonate, child, adolescent, adult, geriatric).
Ensures relevant patient specific parameters are present in the electronic medical record (EMR) and considered (i.e., diagnosis, known allergies, weight, age, etc.).
Engages with other clinical pharmacists or clinical pharmacist specialists for complex clinical situations.
Contacts the prescriber or covering physician, as necessary to initiate appropriate interventions to ensure therapeutic efficacy, minimize potential risks of drug therapy, and optimize cost effectiveness.
Provides accurate and comprehensive drug information and consultation to prescribers, nurses, and other health care professionals regarding appropriate drug therapy, including drug interactions, potential side effects or adverse reactions, incompatibilities, allergies, medication order discrepancies, drug stability, and drug administration.
Identifies, documents, resolves, monitors, and prevents drug therapy related problems, including contraindications, duplications of therapy, and drug interactions.
Documents and reports all clinical interventions, medication errors, near misses, and adverse drug events in accordance with established protocols.
Supervises work performed by pharmacy interns and technicians to ensure accuracy and maintenance of performance standards in accordance with established procedures and federal and state rules and regulations.
Participates in continuous quality assessment and improvement activities and total quality management initiatives, including, but not limited to adverse drug reaction, pharmacist clinical intervention, and medication incident reporting programs.
Participates in the training of other healthcare professionals, including student interns and technicians, physicians, practitioners, nurses, and other professionals on matters relevant to medication use.
Maintains professional competence through participation in Pharmacy staff development and skills enhancement programs, including but not limited to ongoing independent study, and membership and participation in professional organizations.
Demonstrates effective personal, interpersonal, and teamwork skills.
Performs other duties as assigned.
PGY1 PHARMACY:
Receives inpatient patient medication orders or prescriptions as written or ordered by a physician or other practitioner. Reviews and interprets each order to determine completeness. Proactively approves medication orders in the EMR including drug, dose, dose form, frequency, and duration of therapy.
Attends and participates in daily medical, multi-disciplinary, and specialty rounds as time allows.
Participates in emergency code blue response.
Reviews and approves formulations for sterile and non-sterile medications requiring compounding or manipulation prior to dispensing. In accordance with established procedures, completes in-process and final product checks, prior to dispensing medications prepared by pharmacy technicians.
Manages formulary and medication-use processes effectively. Ensures compliance with target drug programs, formulary management initiatives, medication management initiatives, and adverse drug event monitoring programs.

PGY1 COMMUNITY BASED PHARMACY:

Reviews, approves, and enters medication orders in the computerized pharmacy information system including appropriate product formulation, dosage, frequency, and duration of therapy. Ensures relevant patient specific parameters are included on the patient's medication profile (i.e., diagnosis, known allergies, weight, age, etc.). Reviews and approves all medication orders and patient specific parameter computer entries if delegated to technician staff.
Integrates patient-focused medication therapy management and pharmaceutical care in accordance with established clinical pathways or guidelines.
Participates in coordination of transitions of care, as appropriate. Removes barriers to medication access.
Provides education and instruction on use of medications to patients, families, and caregivers as required.

MINIMUM QUALIFICATIONS:

Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy required. Prior to start date, possesses current unrestricted licensure as a licensed pharmacist, licensed pharmacy intern, or as a licensed 90-day temporary pharmacist in the State of Rhode Island.
Prior to October 1supst /sup(or November 1supst/sup if the resident is granted a 30-day extension from the Resident Advisory Committee) of the residency year obtains unrestricted licensure as licensed pharmacist in the State of Rhode Island, then possesses and maintains current unrestricted licensure as licensed pharmacist in the State of Rhode Island.
Must possess and maintain current American Heart Association (AHA) Basic Life Support (BLS) Certification by November 1supst/sup of residency year. Additionally, Inpatient Residents must possess and maintain current AHA Advanced Cardiac Life Support (ACLS) Certification by November 1supst/sup of the Residency Year.

EXPERIENCE:

Clinical experience, knowledge, and development commensurate with a Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
Knowledge of area/specialty specific technology, including, but not limited to, pharmacy information systems, automated dispensing technology software, equipment, and automation. Experience with MS Word, Excel, Outlook, and web browsers.
Excellent written and verbal interpersonal and communication skills.
WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:
Extended periods of time spent standing and walking.
Requires the visual and manual dexterity to operate a computer.
Occasional travel between facilities or sites as required.
INDEPENDENT ACTION:
Performs independently within broad scope of department policies and practices.
Refers specific complex problems to manager where clarification of departmental policies and procedures may be required.
