• Temp-to-hire role
• Eligible for direct hire after 600 hours

• Manage care for a small caseload (only 10 individuals)
• Coordinate appointments, medications, and health services
• Monitor health status and ensure proper care plans are followed
• Work closely with staff, providers, and care teams
• Train support staff on basic health procedures
