Remote Patient Monitoring Care Navigator provides technology-driven solutions to help older adults manage their chronic health conditions through Chronic Care Management (CCM). Our goal is to enhance patient outcomes by offering proactive healthcare management and real-time insights for caregivers and providers.

Job Relationships

Reports to the Operations Manager

Principal Responsibilities

  • Coordinate chronic care plans for patients in CCM and RPM programs.
  • Track patient data from remote monitoring devices to identify trends.
  • Check in with patients (by phone or in-person at the clinic) to assess progress.
  • Communicate important health updates to physicians and clinical teams.
  • Educate patients about medication, lifestyle, and self-management techniques.
  • Document patient interactions and clinical updates in the EHR system.
  • Schedule and oversee follow-up appointments and referrals.
  • Help patients navigate available healthcare services and resources.
  • Escalate any potential risks or health changes to providers promptly.
  • Foster ongoing patient engagement to boost long-term health outcomes.
  • Comply with the Springfield Clinic incident reporting policy and procedures.
  • Adhere to all OSHA and Springfield Clinic training & accomplishments as required per policy.
  • Provide excellent customer service and adhere to Springfield Clinic's Code of Conduct and Ethics Standards.
  • Perform other job duties as assigned.

Education/Experience

  • Educated as a LPN (LVN) by an approved program OR
  • Graduate of an accredited medical assisting program as required by the certifying body.

Licenses/Certificates

  • Current licensure as a LPN in the state of Illinois OR
  • Maintain RMA, CMA or NCMA credentials according to requirements established by the certifying body.
    • Provide copy of credentials to Human Resources prior to expiration of certification period.
    • Must be a registered or certified Medical Assistant through one of the following: American Med Techs (AMT), American Association of Medical Assistants (AAMA), National Center for Competency Testing (NCCT), National Health Career Association (NHA), American Allied Health (AAH), or National Association for Health Professionals (NAHP) at time of hire.
  • CPR certification per American Heart Association guidelines required within 30 days of hire.

Knowledge, Skills and Abilities

  • Familiarity with Chronic Disease Management and Remote Patient Monitoring.
  • Proficiency in using EHR systems for documentation and care coordination.
  • Excellent communication and interpersonal skills to motivate and support patients.
  • Strong organizational abilities to manage multiple patients and tasks effectively.

Working Environment

  • Works in office setting during the hours the Clinic is available to patients/families.
  • Mostly sitting
  • Minimal lifting requirement
