The Hospice Physician is an integral member of the interdisciplinary hospice team, responsible for guiding and overseeing the patient's plan of care. This role works in collaboration with and under the direction of the Hospice Medical Director, supporting the delivery of high-quality, patient-centered end-of-life care.

The Hospice Physician partners with attending providers and serves as a key clinical resource to the care team, ensuring continuity of services and effective management of patient symptoms.

In addition to clinical responsibilities, this individual will act as a champion for the hospice program-supporting outreach and relationship-building with physicians, referral sources, discharge planners, and community organizations to promote awareness and access to hospice services.

Responsibilities

  • 1. Performs all duties in compliance with applicable federal, state, and local laws and regulations and accreditation standards, as well as with the YNHHS/Health at Home code of ethics and policies and procedures at all times. Regular attendance is an essential job function of this position.
  • 2. Supports, demonstrates, and promotes the principles of respectful workplace and core corporate values in all interactions with patients, patient?s family members, other employees, vendors, or visitors.
  • 3. Under the supervision of the Hospice Medical Director, assures continuity and coordination of all Medical Services. Ensures the availability of medical services 24 hrs./day, 7 days/week.
  • 4. Reviews clinical information and at required time frames provides written certification and recertification that patient life expectancy is six months or less.
  • 5. Participation in the development of the plan of care for patients admitted to Hospice.
  • 6. Acting as a liaison to physicians in the community by providing consultation and education to colleagues related to admission criteria for hospice and palliative care.
  • 7. Serving as a resource to the hospice interdisciplinary team and attending IDT meetings (Removed weekly).
  • 8. Adhering to requirements, terms and conditions required by Medicare Conditions of Participation, accrediting body, and federal and state statutes governing the provision of services.
  • 9. Under the supervision of the Hospice Medical Director, assists developing and continually reviewing, in cooperation with the Program Director to monitor the quality of the education programs provided to physicians, personnel, and volunteers.
  • 10. Participates in quality assessment performance improvement (QAPI) plans and monitoring teams and identifying educational needs where identified.
  • 11. Working with the Hospice Medical Director and the Hospice Program Director, to determine the impact of said programs on the quality of care.
  • 12. Assists the Hospice Medical Director in providing training regarding the medical aspects of caring for terminally ill patients to physicians, personnel, and volunteers.

Qualifications

EDUCATION

Currently licensed as a Doctor of Medicine or osteopathy in the state of Connecticut without restriction or subject to any disciplinary corrective action with a minimum of five years of clinical experience in medicine

EXPERIENCE

Have experience in Hospice or palliative care and /or training in end-of-life care and is knowledgeable about the psychological, spiritual and medical aspects of Hospice care.

LICENSURE

Currently licensed as a Doctor of Medicine or osteopathy in the state of Connecticut without restriction or subject to any disciplinary corrective action with a minimum of five years of clinical experience in medicine.
