Summary:
Serves as a liaison between patients and the healthcare team, providing support, education, and care coordination services to help patients navigate the healthcare system and achieve their health goals. Responsibilities:
Participates in the planning and implementation of the plan of care as developed by the Care Manager. Reports changes in patient condition (as identified by the patient) to the Care Manager and/or Primary Care Provider, as appropriate, and supports patient and family education. Maintains a safe environment for patients, families, and visitors at all times. Stays current with Community Health Worker standards, PHS philosophy, and organizational programs to ensure quality care delivery. Supports ongoing professional growth by participating in continuing education, including current literature, in-services, and workshops. Communicates effectively with patients and customers, providing clear verbal and written instructions and information as needed. Participates in local, regional, or state Community Health Worker meetings and events. Performs other duties as assigned.
Qualifications:
High School Diploma or GED required. Completion of specialized short-term training in clinical or medical skills preferred. Ability to communicate effectively in the language(s) spoken by the population served.
