MDS Coordinator LPN

Location: HealthPark Care and Rehabilitation Center,16131 Roserush Ct., Fort Myers FL 33908

Department: Skilled Nursing

Work Type: Full Time (80 hours bi-weekly)

Shift: Days, 8:00:00AM to 4:30:00PM (Monday - Friday) Flexible to meet patient and business needs.

Minimum to Midpoint Pay Rate: $22.34 - $29.04 / hour

This position is responsible for monitoring the nursing care needs of residents and gathering data for the Minimum Data Set (MDS). The coordinator communicates with the interdisciplinary team and collects information necessary for reimbursement and quality reporting related to Medicare, managed care, and long-term care patients. It is essential to ensure the accuracy of MDS and care plans for both Medicare and non-Medicare patients. Additionally, they will coordinate the updating of care plans as needed to reflect the level of care being provided to each resident.

Education: Graduate of an accredited school of nursing required. (LPN)

Experience: 1-year MDS experience preferred.

Certification: RAC-CT required within one year of hire.

License: FloridaLicensed Practical Nurse required.

Other: Excellent communication and customer service skills required.
