Position Title: LTSS Service Care Manager
Work Location: Texas (Dallas, Nueces, and Central SDAs)
Assignment Duration: 12 months
Work Schedule: 8am-5pm
Work Arrangement: Hybrid (field/office with ~75% travel) Position Summary: Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs. Background & Context: This is a backfill for an FTE on a collaborative, member-focused team operating in a fast-paced environment. Key Responsibilities:
* SCs manage member cases through outreach, face-to-face visits, and care coordination.
* The role includes significant fieldwork (about 75% travel), completing assessments, coordinating services, and ensuring members receive needed care while maintaining documentation and compliance standards.
* SCs work a hybrid field/office schedule with 3 travel days and 2 documentation days.
* Travel days focus on completing 2-3 in-person assessments daily.
* Doc days focus on completing required documentation within 3-day TAT, care planning, and follow-ups. Qualification & Experience:
* Required: LVN or LBSW
* Years of experience required: 2-4 years
* Additional qualities to look for: Prior SC/CM experience
