Description

Do you want to apply your clinical skills to make an immediate impact in someone's mental health recovery? Thresholds Service Coordinators compassionately create partnerships with our clients as they transition from resource and housing scarcity to living with dignity in a home of their own.

You will coordinate mental health services at Thresholds, and link to other providers. Each week, you will build short-term yet impactful rapport with new people as you gather information, provide case management services and complete assessments. You may meet people initially in medical or clinical settings across Chicagoland, then as the relationship is built, meet in their homes. You will seek to understand clients' experiences from their perspective and will lower barriers to care by linking clients to appropriate services. This may include medical care, housing, and other resources to help clients achieve and maintain safe, healthy living.

The culture at Thresholds is collaborative and supportive, providing you with the tools and resources you need for the best client care. Opportunities for training and professional development, including clinical supervision, will deepen your knowledge and expand your clinical skills in harm reduction, trauma-informed care, the housing first model and other evidence-based practices.
