Position Title: LTSS Service Care Manager

Work Location: Remote - Florida (Candidates must reside within the state of Florida - Spanish speaking required in one of the following zip codes: 33010, 33147, 33142, 33127, 33192, 33182, 33172, 33127, 33125, 33137, 33128, 33194, 33164, 33174, 33144, 33145, 33155, 33165, 33175, 33183)

Assignment Duration: 12 Months (Contract to Hire)

Work Schedule: Monday-Friday | 8:00 AM-5:00 PM

Work Arrangement: Remote with Field Travel


Position Summary


  • Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes.



  • Develops or assists with personalized service plans for long-term care members and educates members and caregivers on available services and benefits.




Key Responsibilities


  • Evaluate member needs and available resources.



  • Develop and monitor long-term care plans.



  • Coordinate with providers, caregivers, and community resources.



  • Conduct member assessments.



  • Perform home and community visits.



  • Monitor services and identify care gaps.



  • Coordinate referrals.



  • Assist with complaints and appeals.



  • Maintain compliant documentation.



  • Educate members about benefits and services.




Qualifications & Experience


  • Bachelor's degree.



  • 2-4 years of related experience.



  • Strong Microsoft Office skills.



  • Case Management experience.



  • Previous LTSS, LTC, or Hospice experience is preferred.



  • Spanish bilingual preferred for designated territories.

    Must have:

    Strong Microsoft skills

    Nice to have

    Bi-lingual or Tri-lingual would be great but must be able to communicate (verbal and written) in English as well. The members will have intakes in English, but should there be any language barriers that is why the team is seeking someone Bi-Lingual.

    • Previous LTC or Hospice experience
    • Bilingual - Spanish preferred with strong English skills
    • Hialeah to North Miami - preferred. Also 33133, 33130, 33129, 33147



    Previous interview feedback from HM on a general level:

    1. candidates are lacking in answering questions from the team during the interview
    2. Even though the team is seeking bi-lingual candidates - they are noticing that if English is not their first language, they may not be understanding the ask / question
    3. The team needs to ensure the CW can provide adequate and sufficient feedback


    Assessments to members will be completed in English - but with that they are looking to ensure comprehension

    Someone with great time management and great communication and written skills to the hiring manager it is very important for this person to be a team player

    Training is completed virtually and not everyone is a virtual learner - this person should know and be comfortable with that
