Position Summary
We are seeking an Epic Healthy Planet Analyst with strong Population Health experience to support the configuration, optimization, analysis, and ongoing support of Epic Healthy Planet. The ideal candidate will have Epic Healthy Planet certification and experience with population health workflows, risk stratification, quality measures, care management, and value-based care initiatives.

Required Qualifications

  • Epic Healthy Planet certification
  • Strong experience with Epic Healthy Planet configuration, optimization, and support
  • Experience with Population Health workflows and analytics
  • Strong understanding of quality measure logic, including HEDIS and eCQMs
  • Experience with registries, care gaps, risk scoring models, and patient attribution frameworks
  • Experience analyzing and troubleshooting Population Health data and reporting
  • Familiarity with care management documentation and utilization workflows

Preferred Qualifications

  • Experience with Compass Rose and advanced risk stratification tools
  • Experience integrating external data sources, including claims, SDOH, and HIE feeds, into Healthy Planet models
  • Experience supporting payer and value-based contract requirements
  • Experience with Epic Clarity/Caboodle and downstream reporting platforms
  • Experience with Medicare Stars and other quality programs

Key Responsibilities

  • Configure, optimize, and support Epic Healthy Planet build, including registries, risk scoring models, care gap logic, and patient attribution frameworks.
  • Perform detailed analysis of Population Health workflows, including care management, outreach, and quality reporting, and translate operational needs into system design and enhancements.
  • Develop and maintain registries, Reporting Workbench reports, dashboards, and analytics supporting value-based care initiatives and quality programs such as HEDIS and Medicare Stars.
  • Partner with Population Health leadership and clinical teams to identify care gaps and implement system-driven interventions to improve quality and cost outcomes.
  • Design, build, and maintain care management tools, including care plans, protocols, assessment tools, and outreach workflows within Healthy Planet.
  • Support payer and value-based contract requirements by ensuring accurate data capture, attribution logic, and measure calculation within Epic.
  • Analyze and troubleshoot data integrity issues related to Population Health metrics, registry inclusion/exclusion criteria, and reporting discrepancies.
  • Collaborate with data and analytics teams to validate data flows between Epic, Clarity/Caboodle, and downstream reporting platforms.
  • Lead or support optimization efforts to improve user efficiency, including workflow redesign, automation opportunities, and enhanced reporting solutions.
  • Participate in governance and prioritization processes for Population Health enhancements, ensuring alignment with organizational goals and regulatory requirements.
  • Support testing, validation, and change management activities for Healthy Planet upgrades, new releases, and enhancement deployments.
  • Provide end-user support and issue resolution for Population Health stakeholders, including care managers, quality teams, and operational leaders.
  • Assist with onboarding new programs or service lines into Healthy Planet, including registry design, measure mapping, and workflow alignment.
  • Maintain documentation for build standards, workflows, and reporting logic consistent with IT and Population Health governance standards.

  • Cheers/Campaigns.
  • Compass Rose programs.

Schedule:

  • Monday-Friday, approximately 8:00 AM-5:00 PM.
  • Schedule may shift earlier or later as needed.
  • 8 hours per day, 40 hours per week.
