Merakey is seeking a Nurse (LPN) Case Manager to provide community-based services throughout Lehigh and Northampton Counties. This role focuses on supporting adults with both mental health and medical diagnoses through in-home visits, care coordination, health advocacy, and collaboration with interdisciplinary providers. The ideal candidate is passionate about delivering person-centered care and building meaningful relationships that help individuals thrive in their communities.

The LPN Case Manager will be responsible for:


  • Coordinating and implementing services in accordance with each individual's person-centered service plan.
  • Managing and coordinating consumers' overall medical care, including monitoring and following through on physician orders and medication recommendations.
  • Providing health education to promote understanding of medical conditions, treatment plans, and wellness goals, while advocating for consumers during medical appointments.
  • Delivering case management, supportive counseling, and skills training services using a flexible, community-based approach.
  • Participating in an on-call rotation to provide support and respond to urgent consumer needs as required.

Earn: $31.98 per hour.

The ideal candidate will possess the following qualifications:



  • Must be a Registered Nurse, Licensed Practical Nurse or bachelor'sdegree in nursing with a minimum of 12 credit hours in social science.
  • a minimum of 2 years of experience in a mental health setting.
  • Valid PA Driver's License and vehicle for travel to clients-Mileage reimbursement provided
  • Open to working out in the community in Lehigh and Northampton counties Monday-Friday, 8am-4:30pm.
