Clinical Care Liaison

Summary:

The Clinical Care Liaison under the supervision of the Transfer and Access and LifePACT Clinical Manager (or designee) collaborates with the EpiCenter team to enhance care accessibility within the Brown University Health system. This role entails the facilitation and when appropriate the initiation of communication between practitioners at referring institutions and within Brown University Health to coordinate care and service including but not limited to admissions clinic appointments community paramedicine visits and ambulance transport. The Clinical Care Liaison acts as the primary point of contact for clinical and administrative coordination adhering to established departmental policies and procedures.

Responsibilities:

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Respects patient confidentiality and dignity and complies with all HIPPA regulations.

Establishes and maintains communication channels using state-of-the-art communication equipment to facilitate in real-time consultations referrals and patient transportation.

Coordinates with internal and external practitioners to secure and schedule clinic appointments for patients being discharged from a Brown University Health facility based on the patient*s identified needs.

Completes all required documentation with emphasis on compliance and clarity within designated time frames.

Collects and coordinates the dissemination of patient demographic and health insurance provider information to patient registration and business office.

Initiates patient registration process utilizing bed control portal and established protocols. Works collaboratively with bed managers Transfer Center Admitting Department and clinical units to coordinate the appropriate and efficient placement of patients.

Dispatches LifePACT team as directed by Transfer and Access Coordinators. Maintains communication via radio/phone with LifePACT team. Tracks and documents all required response times.

Works in collaboration with case managers and community paramedics to coordinate and schedule community paramedicine visits. Serves as liaison between community paramedics in the field and the necessary internal services during home visits.

Monitors and oversees the Roundtrip system for scheduling ambulance services. Assists in the coordination of ambulance needs for the Brown University Health system. Resolves any issues that may arise in real time with ambulance services.

Effectively communicates relevant information to managers and medical directors. Proactively raises awareness of issues that require escalation.

Ensures proper functioning of all communication and support equipment.

Participates in community outreach and in-house educational efforts such as grand rounds departmental staff meetings to promote awareness and education.

Meets and maintains mandatory safety education and clinical competencies based on pre-determined credentialing standards.

Maintains collaborative relationships with team members other members of the health care system and outside agencies including but not limited to transferring and receiving hospital customers EMS personnel and community agency representatives.

Utilizes effective interpersonal skills to identify and resolve conflicts reduce communication barriers and facilitate group effectiveness.

Participates in Quality Assurance activities both in organized meetings and through measurement of operational and clinical quality indicators for department.

Participates and assists in the orientation of new employees as assigned.

QUALIFICATIONS:

Must have current licensure to practice as a medical assistant or emergency medical technician (EMT) in the State of Rhode Island. Must successfully complete any associated credentialing specific to Referral Care Coordinator.

Minimum of 1 year of current experience as a medical assistant or emergency medical technician.

Prior experience within hospital and/or EMS environment is preferred.

Strong computer skills with proficiency in in Microsoft Excel Outlook PowerPoint Access and Word. These skills will be utilized for tasks such as retrieving demographic information analyzing performance data and generating statistics.

Proficiency in operating transmission equipment including two-way radios.

Excellent verbal and written communication skills to effectively collaborate with transport staff physicians and other hospital personnel and patients.

Demonstrated ability to provide exceptional customer service and exhibit positive customer service behaviors.

Must be flexible highly organized and able to work in a fast-paced environment.

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:

Visual and auditory acuity to effectively work with computer systems and transmission equipment.

Ability to sit for extended periods of time to perform job duties.
