Position Highlights:

  • Position: Behavioral Health Integration Clinician I

  • Department: Behavioral Health Integrations and Primary Care

  • Location: Foster Medical Pavilion. 5215 N California Ave Suite 603, Chicago, IL 60625

  • Hours: Generally 8am-4:30pm with some flexibility in start/end time

  • Days: Day Shift- Monday thru Friday

  • Part time/Full Time: Full Time 40 hours per week

What you will need:

  • Master's Degree or higher in a related mental health field

  • Current State of Illinois licenses such as: Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) within six months of start date into position

  • CPR

  • Illinois Driver's License

  • Preferred a minimum of one year or more of experience in mental health setting or healthcare setting.

General Summary:

A Behavioral Health Integration Clinician I is a core member of the BHI and primary care team which includes the medical provider, psychiatric consultant and the larger patients care team within the Endeavor Health system. Responding to BHI orders to engage patients in BHI services or help connect with specialty mental health services through a BHI Service Consult, supporting and coordinating the mental health and related physical health care of patients referred to BHI utilizing the Collaborative Care Model. May also have some opportunity for providing individual therapy.

Provides collaborative care to patients when referred by Endeavor Medical Group physicians/providers, ensuring their access to mental health and substance abuse services across all care levels including but not limited to BHI, Counseling Center, inpatient, outpatient group therapy, and medication management.
