Salary: $110,000-$135,000 annually
Range based on education and experience

Schedule: Full-Time | Every other Friday off (paid wellness day)

This position is eligible for a $10,000 sign on bonus with a two-year employment commitment.

Your Role:

Advanced Practice Nurse - Primary Care - Heritage Behavioral Health Center (Decatur, Illinois)

The Advanced Practice Nurse (APN) will work with the primary healthcare team to ensure individuals with mental illness or substance use diagnoses receive high-quality primary medical care within the standard of care for Internal Medicine patients. This role combines clinical expertise with compassionate care to improve health outcomes for our clients.

Core Responsibilities:
  • Perform physical examinations of patients to evaluate health and diagnose illnesses
  • Determine, plan, and initiate appropriate treatment, adjusting as necessary
  • Prescribe and dispense medications
  • Document medical information in electronic health records
  • Educate and counsel patients and families on health problems, treatment plans, and home care
  • Encourage and promote preventive health care and positive health behaviors
  • Request consultations or referrals with other healthcare providers
  • Maintain knowledge of current medical trends, including medication use and side effects

Qualifications:
  • Graduate degree from an accredited nursing school (Master of Science in Nursing)
  • Current licensure in the State of Illinois (in good standing and without prior disciplinary action)
  • Valid driver's license, reliable transportation, and proof of insurance
