Requisition ID req31317 Working Title Program Manager Position Grade 13 Position Summary Pathways to a Healthier Bernalillo County is a multi-stakeholder SDOH intervention program funded through the Mill Levy. This program supports community partners and organization to employ Pathways Navigators who provide high risk and vulnerable populations with case management services, resource links, and access to ameliorate adversity and improve health outcomes. Currently, there are 17 Pathways Navigator organizations throughout the county. The Program manager will serve as the primary point-of-contact and oversee all program services, deliverables, and administration of the Pathways Program. This role is responsible for meeting deliverables, ensuring execution of service contracts, and reporting to the program advisory group and organizations. Conditions of Employment Minimum Qualifications High school diploma or GED; at least 6 years of experience 2 of which are management level experience directly related to the duties and responsibilities specified. Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. Preferred Qualifications -Experience with contracts and budget management. - Experience overseeing a large, multi-faceted program. - Experience working with Community Health Workers, Peer Navigators, Peer Support Workers, etc. - Experience with social or community health services. - Bilingual preferred. - Bachelor's degree in business administration, community health, public administration, or related field. - Experience with SmartSheets, purchasing, invoicing, reconciliation, and Pcards. - Experience in reflective supervision.
